Insurance Information

We are a network provider for TriCare, Anthem Blue Cross Blue Shield, Cigna, and Aetna. (Not all plans from all companies cover tele-mental health services. You should check with your insurer to determine coverage for your particular plan.)

Payment Information

For charges not covered by insurance or subsidy, payment is expected at time of service. MasterCard, VISA, Discover, American Express, check, Ivy Pay, and PayPal are accepted.

 
Forms-Fill out and bring to first appointment.

Our intake forms are available to be filled out via a secure online connection. After you have spoken with Kay and you have a scheduled appointment, you will receive an email with instructions for filling out the required forms. This can typically be done on a computer (Mac or Windows), iPad, iPhone, or most Android devices. The forms listed below are not required unless you receive specific instructions to the contrary.
